Epicardial coronary artery may enter into the heart muscle in some areas and be compressed by muscle fibers. This is termed as myocardial bridging. Although previously considered to be benign, rarely it may cause serious clinical pictures such as angina, myocardial infarction, arrhythmias and sudden cardiac death. Treatment is medical and surgical. We presented a patient with acute coronary syndrome due to myocardial bridging of left anterior descending coronary artery.
